While we do accept insurance, not all our clinicians accept all insurance plans. Interns cannot accept insurance until they graduate and operate on a sliding fee. We can provide a superbill if you would like to submit it to insurance yourself, and we will continue to take FSA/HSA cards for payment. Please note that this form is not a guarantee of services, and if you are accepted, additional paperwork will be sent to you when we contact you.
